Home for Life Design® is committed to protecting your privacy and developing technology that gives you the most powerful and safe online experience. This Statement of Privacy applies to the Home for Life Design Website application and/or mobile application and governs data collection and usage. By using the Home for Life Design website application and/or mobile application, you consent to the data practices described in this statement. For the purposes of this document, “client” as used herein refers to an individual receiving home design services; “user” as used herein refers to one or more service providers providing home design services to the client.

 

Information Voluntarily Provided

Home for Life Design may collect personal information including your name, email, phone number and any other information that a user knowingly and actively provides when using or participating in any of Home for Life Design’s services. Submitting voluntary information constitutes a user’s consent to the use of the information for the legitimate business purposes described in this statement.

 

Information Automatically Collected

There is information about a user’s computer hardware and software that is automatically collected by Home for Life Design. This information can include a user’s: IP address, browser type, domain names, access times and referring Web site addresses. This information is used by Home for Life Design for legitimate business purposes including to operate the service, to maintain quality of the service, and to provide general statistics regarding use of the Home for Life Design Web site. Please keep in mind that if a user directly discloses personally identifiable information or information that is personally sensitive, this information may be collected and used by others. Data Gathering

The application was designed to meet the PHI (Protected Health Information) De- Identification standards of HHS (U.S. Department of Health and Human Services) Safe Harbor provisions §164.514(b)(2). The application therefore does not prompt for any of the personal information belonging to the user’s clients such as name, address, e-mail, phone number, etc. The user is responsible for ensuring no personally identifiable information belonging to the user’s clients is entered into any of the text fields or uploaded images.

 

Use of Cookies

In some countries, Home for Life Design is prohibited to collect personally identifiable information via cookies, without your expressed consent. This section assumes that if such requirement is necessary, Home for Life Design has received such expressed consent.
